Section 12-220-401
Application for dentist license

(1)
Every person not currently holding a license to practice dentistry in this state who desires to practice dentistry in this state must file with the board an application for a license on a form provided by the board, verified by the oath of the applicant, and accompanied by a fee established pursuant to section 12-20-105, indicating that the applicant:(a)
Has attained the age of twenty-one years;(b)
Is a graduate of a dental school or college that, at the time of the applicant’s graduation, was accredited. An official transcript prepared by the dental college or school attended shall be submitted to the board.(c)
Has listed any act the commission of which would be grounds for disciplinary action under section 12-220-201 against a licensed dentist, along with an explanation of the circumstances of the act;(d)
Has proof that the applicant has not been subject to final or pending disciplinary action by any state in which the applicant is or has been previously licensed; except that, if the applicant has been subject to disciplinary action, the board may review the disciplinary action to determine whether it warrants grounds for refusal to issue a license; and(e)
Has proof that the applicant has met any more stringent criteria established by the board.(2)
An applicant for licensure must demonstrate to the board that the applicant has maintained the professional ability and knowledge required by this article 220 when the applicant has not graduated from an accredited dental school or college within the twelve months immediately preceding the application and has not, for at least one year of the five years immediately preceding the application, engaged in:(a)
The active clinical practice of dentistry;(b)
Teaching dentistry in an accredited program; or(c)
Service as a dentist in the military.(3)
The board may require other pertinent information on the application that the board deems necessary to process the application, including demonstration of compliance with the financial responsibility requirements set forth in section 13-64-301 (1)(a).
